Geographical Location
Schwanheide is located in western Mecklenburg-Western Pomerania, about six kilometres northwest of Boizenburg/Elbe, and borders Schleswig-Holstein.

The Mühlenbach flows through Schwanheide. The Riedebeck follows the municipality's northwestern boundary, while the Delvenau flows through the Stecknitz-Delvenau nature reserve towards the Elbe, running along the western boundary.
